What work-up is needed prior to the first appointment?

Please provide us with any office notes, labs and/or diagnostic images that pertain to the problem that are not accessible through CHKD's Electronic Health Record (EHR). This information can be faxed to (757) 668-7721.

Where will my patient be seen?

The adolescent medicine-gynecology offices are located at CHKD (Norfolk), Lightfoot (Williamsburg) and Oyster Point (Newport News).
