How can I refer my patient to CHKD's developmental pediatrics practice?

When will my patient be able to obtain an appointment?

Your patient's estimated wait time for an appointment is approximately nine (9) months, but could vary depending on location.

What work-up is needed prior to the first appointment?

We will send you a new patient packet that includes a check list, new patient comprehensive history form, and Vanderbilt Parent and Teacher rating scales. These forms must be completed and mailed back to our office to move forward. If we do not receive the completed packet within 30 days, your child will be removed from the wait list.

We schedule appointments approximately 2-3 months in advance. We will contact you to offer a specific appointment time about two months prior to appointment. You are welcome to contact us with any questions or to make sure we received your packet and have your up-to-date contact information.

Where will my patient be seen?

Developmental Pediatrics offices are located at Butler Farm (Hampton), Children's Medical Tower (Norfolk), Harbour View (Suffolk), and Concert Drive (Virginia Beach).
