NORFOLK, VA - All eighteen pediatric practices that make up the CHKD Medical Group have been awarded the highest level Patient-Centered Medical Home (PCMH) Recognition, Level 3, from the National Committee for Quality Assurance (NCQA). This recognition is awarded to primary care medical practices for the use of evidence-based, patient-centered processes that focus on coordination of care and long term provider-patient relationships. As only one in six primary care physicians in America practices in an NCQA-Recognized practice, the 120 pediatricians in the CHKD Medical Group are now among an elite group that has demonstrated its commitment to advancing quality in healthcare.
The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to enhance care, improve patient experiences and reduce costs. “As a medical home, it is the role of each CHKD pediatric practice to serve as a home base for all their patients’ healthcare needs by keeping a comprehensive medical history, coordinating all care and working in
partnership with patient families to ensure that both medical and non-medical needs are met,” says Dr. Doug Mitchell, a pediatrician at General Booth Pediatrics and medical director of the CHKD Medical Group. Research has shown that medical homes can lead to higher quality, lower costs and can improve the healthcare experiences of both patients and providers.
“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” says NCQA President Margaret E. O’Kane. “Recognition shows that each of the CHKD pediatric practices have the tools, systems and resources to provide their patients with the right care, at the right time.”
To earn recognition, which is valid for three years, each CHKD pediatric practice demonstrated the ability to meet the program’s key elements, embodying characteristics of the medical home. NCQA standards aligned with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.
