Please send any relevant records not accessible through CHKD’s Electronic Health Records (EHR) such as lab results, radiology imaging and other accompanying reports. Some conditions require pre-consult work up. Please contact our office at (757) 668-7992.
When will my patient be able to obtain an appointment?
Your patient will be seen at the earliest appointment available, which is usually within 1-to-2 weeks.
Urgent appointments will be accommodated within 24 hours or less.
